DISCLOSURE STATEMENT

Pastoral Counseling Service (PCS) is a Christian based ministry with a strong commitment to providing quality, professional services that are guided by Christian values and ethics.


BENEFITS AND RISKS OF THERAPY:

Research has shown that therapy can help with a wide variety of problems for children, adolescents, and adults. However, because therapy is a cooperative effort between client and therapist, participation in therapy does not guarantee problem resolution or that the client necessarily will feel better. As with any treatment, whether psychological or medical, therapy should be entered into with appropriate consideration of the potential benefits and risks. If you have any questions about what to expect from therapy, please ask your therapist.


CONFIDENTIALITY:

Records and information collected about clients will be held and released in accordance with state and federal laws governing confidentiality of client records and information. Disclosure of information regarding services provided to the client is generally released to another party only with the client's written permission. Exceptions to this rule include the following cases: (1) when there is imminent danger to the client or another person, (2) when child abuse or neglect is suspected, (3) when disclosure must be made to medical personnel in a medical emergency, and (4) when the therapist is compelled by law to disclosure client records or information. 

Client information and records are available to agency personnel for purposes of research, report, supervision, and quality assurance review.


STAFF CREDENTIALS:

All counselors at PCS possess Master's or Doctoral level clinical degrees and competence in individual, group, marital, and family counseling. As a commitment to standards of excellence in the area of counseling, the PCS counselors adhere to state statutes regulating their discipline and the standards of practice mandated by their professional and regulatory agencies. 

Dr. Ryan Fraser holds a Master of Ministry, Master of Divinity, and Ph.D. in Pastoral Theology & Pastoral Counseling. He is a fellow of the American Association of Pastoral Counselors (AAPC) and is a Certified Pastoral Counselor. Dr. Fraser is a Licensed Clinical Pastoral Therapist (LCPT) in the state of Tennessee. CLIENTS WITH DISABILITIES:

It is the policy of PCS to accommodate clients with disabilities, pursuant to federal and state law. Any client with a disability who needs accommodation should inform PCS staff prior to receiving services. 


NON-DISCRIMINATION POLICY:

In accordance with Title VI of the Civil Rights Acts of 1964, PCS does not discriminate against participants or clients on the basis of race, color or national origin. Services are offered to all eligible persons.


FEE POLICY:

The fee for counseling services provided through PCS is based on a sliding scale according to clients' family annual gross income. PCS does not accept medical insurance and is strictly on a client-pay-for-service basis. You are expected to pay at the time of service. There is a $35.00 fee for returned checks.

The payment sliding scale based on annual family gross income is as follows:

            $0-29,000......................................................$50 per session

            $30,000-39,000..............................................$55 per session

            $40,000-49,000..............................................$60 per session

            $50,000-59,000..............................................$65 per session

            $60,000-69,000..............................................$70 per session

            $70,000-79,000..............................................$75 per session

            $80,000-89,000..............................................$80 per session

            $90,000-99,000..............................................$85 per session

            $100,000-above..............................................$90 per session

For intake sessions, due to lasting 1.5 hours, clients are charged the sliding scale fee x 1.5. (Example:  If your gross family income is $0-29,000, the cost would be $50 x 1.5 = $75)
